Looking for a new mouse for planetside 2 and other games. I would like something that has a thumb and ring finger button as well as the standard load out (right left clicks, middle click and wheel.). Needs to be wired seeming how I sit a good 10ft away from my pc and normal wireless mice never seem to cut it. Would like to keep around or below $40. Seems everything I look at has all or a bzillion buttons on one side and none on the other, or are way too expensive. I know Microsoft made a mouse a while ago that is exactly what I want, however I am unable to find them now (I think they stopped making them.) Any recommendations on a good gaming mouse pad would be cool to.

And WOW, newegg.com has gone to hell!

    LOGITECH G500
is still your best bet. It's above BUT close to your price range on NewEgg. But I'm sure that you can find a better price at time if you look around. It had all the whistles and bells for a low price. 3 programmable thumb triggers, a DPI trigger at the pointer finger, the scroll wheel has three programmable function left right and depressed.
Logitech is a great company with great products.

The sales and marketing team there are very good to me. I've been through two G500 over the years and without hesitation they have replaced each one without question. One time I boxed up the mouse and shipped it to them without calling them first. The contacted me and replaced it without checking a worriment or anything. I'm not saying they will do the same for everyone but they sure did treat me good.

Kern is right, that is a good mouse. If you are willing to go higher, I would recommend investing in a razer naga. Theyre bad ass

    Logitech G400 (the new MX518)

The only thing that will never change in my setup is my mouse. I use an MX518 and I wouldn't work a day without it.

This is what came with my computer: http://www.asus.com/Keyboards_Mice/GX800/
You wouldn't think a mouse that comes with a PC would be good, but it's actually turned out to be great.

2 buttons on the left side, each placed for perfect pressing by your thumb. Simple and effective, and comfortable as well. And the little button in the middle is great for turning hax on and off
